• PEEF income affidavit on Rs.20/- of father’s monthly income with signature is attached.
• Govt. private servant/govt. present servant has attached certified copy of previous pay slip or pension book.
• Graduation Degree/ result card copy is attached.
• Passport size picture with blue background has been glued on the front page and another one has been attached with the form.
• Current students copy of the hostel wardens certified form
• Certificate of the related data on special quota is attached.
• Paid utility bills copy has been attached.
• Attach Copy of Identity card of mother and father.
