Provisional Admission Forms

Please download relevant Provisional Admission Form, print, fill profile data, and bring along with original documents on Orientation Day or 1st Day of semester Start.

Fee Concession Form

1. Please download, print and fill relevant Fee Concession Form as per your Eligibility. Arrange and attach required documents and bring original documents on Interview Day.

Affidavit

• Please download, print and fill AFFIDAVIT (on plain paper) and upload on your documents portal. There is no requirement to print it on stamp paper. Bring original on Interview Day. (Download here)

Instructions for Uploading Documents

1. Scan the original documents (mentioned below) in Jpg/Jpeg format
2. Use a scanner to scan documents. Images also taken through Camera/Mobile.
3. Be sure, each file size does not exceed 20MB.
4. The Admission Office will verify all the uploaded documents. Remarks will be provided in case of rejection.
5. Document cannot be altered once it is verified. In case of correction, contact the admissions office of your respective campus.
6. If you have any problem/query regarding uploading documents. Please contact the admissions office of your respective campus.

Note:   Sufficient time is given to arrange and attach required documents so that candidates do not leave the campus during interview process to arrange these and secure their seat on same day.
